چکیده

We present multi‐modal imaging of two phakic patients who developed lens nucleus dislocation following multiple complicated vitrectomy surgeries. We feel vitreoretinal physicians should be aware of this phenomenon and its presentation. To the best of our knowledge, high‐quality multi‐modal imaging of this presentation has not been published. Two phakic patients with moderate nuclear sclerosis underwent 25‐gauge pars plana vitrectomy (PPV). The first case underwent six PPV procedures; an initial macular hole repair was later complicated by rhegmatogenous retinal detachment, proliferative vitreoretinopathy, tractional retinal detachment and suprachoroidal hemorrhage. The patient noted a sudden change in vision on day 15 after the last surgery and posterior dislocation of the lens nucleus was detected. The second patient underwent PPV for traumatic vitreous hemorrhage, during which a retinal tear was found and repaired. This patient noted a sudden change in vision 3 months after the surgery. Perfluorocarbon heavy liquids in the management of posterior dislocation of the lens nucleus during phakoemulsification.

We report a case in which the lens nucleus dislocated into the vitreous cavity through a posterior capsular rupture during phakoemulsification. We performed a vitrectomy and removed the lens nucleus using the perfluorocarbon heavy liquid perfluoro-1,3-dimethylcyclohexane. The management of posterior dislocation of the lens nucleus during cataract surgery is discussed.

Changes in Anterior Segment Parameters Following Pars Plana Vitrectomy Measured by Ultrasound Biomicroscopy (UBM)

Despite being a posterior segment ocular surgery, pars plana vitrectomy (PPV) may also affect anterior segment parameters. We aimed to investigate anterior segment alterations following PPV using ultrasound biomicroscopy (UBM) to visualize the structural anatomy of the eye. In this study, we enrolled phakic patients undergoing PPV (as the sole procedure). The anterior chamber depth, crystalline...

Traumatic Dislocation of Posterior Chamber Phakic Intraocular Lens into the Berger's Space

Dear Editor, Berger’s space is a potential compartment that can present behind the crystalline lens but is not open in normal situations. This space is formed by the posterior lens surface and anterior vitreous face and is circumscribed by Weiger’s ligament [1]. Except for some congenital abnormalities, Berger’s space is rarely involved in ocular disease because of vulnerability of structures s...

Treatment of a dislocated lens by transcorneal vitrectomy and bimanual phacoemulsification

BACKGROUND As a method of treatment for a dropped lens nucleus, which occurred during cataract surgery, the dropped lens nucleus was removed through the corneal wound without using pars plana vitrectomy (PPV). After vitrectomy, the dropped lens nucleus was floated on the perfluorocarbon liquid (PFCL). The floating lens nucleus was then phacoemulsified and aspirated. During surgery, irrigation f...

Implantation of foldable posterior chamber intraocular lens in aphakic vitrectomized eyes without capsular support Implantação lente intraocular dobrável de câmara posterior em olhos afácicos vitrectomizados sem apoio capsular

The capsule of the crystalline lens, if sufficiently present, is used as a support tissue for intraocular lens (IOL) implantation surgery performed on vitrectomized eyes. Eye trauma or iatrogenic damage during complicated cataract surgery, loss of position and zonule connections of the crystalline lens, and IOL via dislocation may cause loss of support tissue for lens capsule implantation prior...
